About the Role:
As part of our continued investment in services for care experienced young adults, we are seeking an experienced, passionate and ambitious Senior Personal Adviser to take a lead in working with partners to plan effective transitions for care leavers aged 16-25. The role will focus on those care leavers with complex needs; vulnerable to exploitation, significant mental health, domestic abuse/violence and those with significant and additional health needs.
This is a unique opportunity to shape and influence outcomes for some of our most vulnerable young adults by providing strategic coordination across children's services, adult social care, health, housing, mental health services, education, employment providers and wider community partners.
The successful candidate will act as the lead coordinator for complex transitions, ensuring that care experienced young people receive timely, joined-up and effective support as they move into adulthood. You will champion their voice, promote positive outcomes and ensure that no young person experiences unnecessary barriers due to fragmented service delivery.
Key Responsibilities:
In this role, you will:
Lead Multi-Agency Coordination
- Act as the designated lead coordinator for care leavers with complex and multiple needs requiring enhanced intervention from partner agencies.
- Chair and coordinate multi-agency planning meetings, professional networks and complex case discussions.
- Develop robust transition pathways between Children's Services and Adult Services, ensuring clarity of roles, responsibilities and accountability.
- Escalate barriers to service access and work proactively with partner agencies to secure appropriate support and resources.
Support Care Experienced Young Adults
Work to support practitioners working with care leavers aged 16-25:
- Who have significant vulnerabilities, disabilities, neurodiversity, mental health needs, learning difficulties, substance misuse issues, exploitation concerns or other complex challenges.
- Build trusted, relationship-based practice that promotes independence, resilience and long-term stability.
- Complete regular visits and maintain effective Pathway Plans that reflect the young person's aspirations, strengths and identified needs.
- Advocate for young people and ensure their views, wishes and feelings remain central to all planning and decision-making.
Drive Practice Excellence
- Provide consultation, guidance and reflective support to Personal Advisers across the service on complex cases.
- Lead workshops, learning opportunities and practice forums to strengthen understanding of transitions, adult services pathways and complex needs.
- Promote best practice across the Care Leavers Service through coaching, mentoring and professional leadership.
- Support quality assurance activities and contribute to service development and continuous improvement.
Help to Strengthen Strategic Partnerships
- Develop and maintain effective partnerships with:
- Adult Social Care
- Adult Mental Health Services
- Learning Disability Services
- Housing Providers
- Integrated Care Boards and Health Partners
- Education, Training and Employment Services
- Substance Misuse Services
- Specialist Community and Voluntary Sector Organisations
- Represent the Care Leavers Service at multi-agency forums and strategic meetings.
- Influence service development and help establish effective pathways that improve outcomes for care experienced young adults.
What We Are Looking For:
We are looking for a highly motivated professional with:
- Significant experience working with care leavers, vulnerable young adults or young people with complex needs.
- Strong knowledge of care leaver legislation, statutory guidance and transition planning.
- Proven experience of leading and coordinating complex multi-agency interventions.
- Excellent relationship-building, negotiation and influencing skills.
- A sound understanding of Adult Social Care, Mental Health Services, Housing and Health systems.
- Experience of supporting practitioners through mentoring, coaching or reflective practice.
- A strong commitment to trauma-informed, relationship-based and strengths-focused practice.
- The ability to challenge appropriately, advocate effectively and achieve positive outcomes for young people.
