    Family Help Children with Disabilities (CWD) Preparing for Adulthood

    The Redbridge CWD Service has been identified as Council Priority Area within the Corporate Plan. The Service is therefore undergoing a transformation, which includes the creation of a Preparing for Adulthood team to improve the timeliness of transition planning and the quality of the experience of disabled young people preparing for adulthood and moving from Children's Services into multi agency adult services provisions.

    We are very excited to present a unique opportunity for the permanent role of Preparing for Adulthood Senior Family Support Worker, a joint Children and Adults funded Social Care role held within Children's Services. Within this role you will be mainly working with young people from 16 to 18 years of age however, there may be opportunities to work with younger people. Your main role will be to support disabled young people in their transition from Children&©s Social Care to Adult Social Care and the Leaving Care Service, complete Transition Assessments, Care and Support plans and Mental Capacity assessments. You will be working within and across the remit of the Children Act 1989, the Care Act 2014, the Children and Families Act 2014 and the Mental Capacity Act 2005. You will work closely with a variety of professionals including Adult Services, Mental Health Services, an array of health professionals, and colleagues from Education (SEND).

    You will be working with young people to co-ordinate what their adult package of support should look like and promote their wellbeing as they transition into early adulthood.

    The right candidate will need to demonstrate strong professional values of promoting personal dignity, advocating to promote physical and mental health and emotional wellbeing. You will need to be passionate about autonomy and the elevation of personal control of an individual over their day-to-day life. An understanding of the principles of protection from abuse and neglect for both children and adults is essential.

    We are looking to recruit an aspiring Social Care worker who is ready to take the next step in their career progression as a Preparing for Adulthood Worker, who can manage a caseload consisting mainly of young people progressing to adulthood. The variety of work will provide you with an interesting and challenging experience and the opportunity to obtain a vast repertoire of skills that will enhance your practice. You will be part of a friendly, stable, aspirational team, which has a positive team morale. You will have opportunities to develop through training and co-working. The Service Area has built a strong learning environment through their Student Academy which has received positive recognition from several universities; therefore, you will have the opportunity to work with Student Social Workers/Occupational Therapists, contributing to their development and experience.

    The ideal candidate should have the following attributes:

    • Substantial experience of working with young people and their families.
    • Excellent writing skills and confidence in representing the service at professional forums.
    • Committed to achieving positive outcomes for young people.
    • Experience of completing assessments and developing SMART care plans for young people and their families.
    • A proven track record of establishing positive and meaningful relationships with young people and their families as well as colleagues.
    • Able to demonstrate personal and professional autonomy in relation to practice.
