Join our Early Intervention and Prevention Team on a 12-month fixed-term contract, where you will use your professional judgement, creativity and strengths-based approach to help residents achieve the outcomes that matter to them.

About you…
As a qualified, HCPC registered Occupational Therapist within our Early Intervention and Prevention Team, you will help people maximise their independence, remain safely in their own homes and communities, and achieve the outcomes that matter to them.
With relevant experience in occupational therapy that includes manual handling you will be resilient and organised and confidently manage a varied caseload, make sound decisions and remain calm under pressure.
You will carry out holistic assessments, setting meaningful reablement goals and developing personalised support plans. Working closely with individuals, carers, colleagues and partner organisations, you will use your professional expertise to support timely hospital discharge and help prevent unnecessary hospital admissions.
You will be committed to person-centred, strengths-based practice and working collaboratively to promote independence and wellbeing.
About the team…
You will join our Early Intervention and Prevention Team, working alongside Occupational Therapists, Physiotherapists, Nurses, Social Workers and other colleagues, while benefiting from regular supervision, opportunities to develop your practice and flexible working arrangements.
Based at Times Square in Bracknell, we work collaboratively in a fast-paced environment, with a strong focus on reablement, professional development and reflective practice.
Key responsibilities…
- Manage your own caseload, completing timely, person-centred assessments, reviews and reablement plans.
- Work collaboratively with individuals, carers and multidisciplinary colleagues to develop SMART goals that maximise independence, choice and control.
- Act as an autonomous practitioner, maintaining accurate records and contributing to safeguarding assessments where required.
- Support timely hospital discharge, prevent avoidable admissions and help people remain safely within their communities.
- Participate in team meetings, mandatory and specialist training, supervision and reflective practice, with opportunities to develop your skills and support or mentor colleagues and Occupational Therapy students.
