This role is to provide cover while an existing member of staff undertakes a secondment, ensuring the continued delivery of high-quality services to Croydon residents.

Live Well is a healthy lifestyle service designed to reduce health inequalities and increase life expectancy by supporting residents to make positive, sustainable changes. You will work with residents on smoking cessation, weight management and alcohol reduction, guiding them through a structured behaviour change journey using both digital tools and personalised support.

This is a rewarding, front-line role working directly with residents in the community, empowering individuals to identify solutions and achieve their personal health goals.

Key Responsibilities

  • Deliver 1:1 and group behaviour change interventions through face-to-face, video and telephone sessions
  • Carry out initial assessments, goal setting, and regular reviews as part of structured 12-week programmes
  • Support residents with smoking cessation, including the provision of approved nicotine replacement and pharmacotherapies in line with national guidance
  • Provide brief and extended brief advice around alcohol and weight management
  • Manage your own caseload and maintain accurate, timely electronic records
  • Deliver community engagement sessions, including MECC (Making Every Contact Count) awareness sessions
  • Promote the Live Well service across internal and external partner organisations
  • Work collaboratively with GP practices, pharmacies, voluntary organisations and community services

About You

You will bring:

  • Experience delivering behaviour change programmes in at least one of the following: smoking cessation, weight management or alcohol support
  • Strong motivational interviewing and coaching skills
  • Excellent communication skills and the ability to build strong, trusting relationships
  • Confidence working both independently and as part of a team
  • Strong IT skills and the ability to confidently use client management systems
  • Good organisation and time management skills
  • An understanding of health inequalities and barriers faced by local communities

Desirable (or willingness to work towards):

  • NCSCT Level 2 (Smoking Cessation)
  • Healthy Weight Coaching qualification
  • Alcohol Extended Brief Intervention certification

What We Offer

  • The opportunity to make a real difference to residents’ health and wellbeing
  • Supportive team environment with access to training and development
  • Experience working within a respected local authority public health programme
