    The Role:

    • By becoming a leader of the Independence and Reablement team you’ll be playing a key role in helping people to achieve independence, relearn skills and build confidence, empowering people to live physically, emotionally, and socially full lives reaching their aspirations,
    • As an Independence & Reablement Team Leader (Registered Manager) you will manager a team of staff who will use a strengths-based approach, to work directly with individuals over a short and focused period to develop person-centred goal plans and ensure holistic support is given.
    • You will be a Care Quality Commission (CQC) Registered Manager for the Independence and Reablement Service, ensuring the service is effective, efficient and safe at all times. As a Registered Manager, the Team Leader is responsible for ensuring that the Fundamental standards of the Health and Social Care (Regulated Activities) Regulations 2014 and Care Quality Commission (Registration) 2009 Regulations are maintained to a good standard.
    • You the Independence & Reablement Team Leader will have an active role within the Independence and Reablement Multidisciplinary Team to coordinate strengths-based assessments, support planning, and resource allocation.
    • Within your role the Independence & Reablement Team Leader will create a culture of positive performance management. Ensure robust performance management of the service by owning and developing service planning, budget management and performance measurement.
    • Independence and Reablement Team Leaders will develop effective partnerships with colleagues across Liverpool City Council and external partners to support a holistic, efficient, and person-centred service, focused on maximising a person’s independence
