About the team

Brokerage Solutions Team is committed to sourcing services for people supported by Hertfordshire Adult Care Services colleagues. We are looking for a hardworking, resilient, passionate, and experienced person to apply for the post of Home Finder and join our dynamic team.

Brokerage Solutions Team provides support to Social work professionals who are working with individuals who have been assessed as meeting criteria to access social care support for community and residential services. As a Home Finder you will predominately be dealing with long stay placement requests but also will be involved in Prevention of Hospital Admissions and respite requests.

This role requires strong communication skills, as you will work closely with both internal and external stakeholders, including Functional Mental Health Teams, hospitals, and care homes, as well as maintaining direct contact with clients and their family members.

You will be committed to delivering a high-quality service within a demanding and fast‑paced environment. You must be fully up to date with the required IT skills or demonstrate the ability to learn new systems quickly. The role also requires the ability to prioritise cases effectively and work under pressure.

The Brokerage Solutions Team operates on a hybrid working model, with team members working from the office a minimum of two days per week and the remainder from home.

About the role

  • Managing and prioritising caseload.
  • Commissioning services accurately
  • Sourcing appropriate services and liaising with Care Managers as well as direct contact with clients and families.
  • Working collaboratively with stakeholders to meet request in timely manner and within the budget set.

About you

Essential:

  • Experience of working in Social Care and understanding of the Care Act.
  • Strong communication skills with the confidence to speak directly to clients and family members, both verbally and written with the ability to establish strong relationships with providers.
  • Must be self-motivated and able to work in a fast-paced, pressured environment.
  • Working as part of a team and individually
  • Excellent organisational, IT and time-management skills.
  • Knowledge of Microsoft applications.
  • Ability to travel to one of the two main office bases twice a week.

Desirable:

  • Outstanding organisational and prioritisation skills
  • Knowledge of Brokerage
  • Knowledge of ACSIS.
