Job introduction
We are looking for a Higher Grade Rehabilitation and Reablement Assistant to join our Community Rehab Service to support services users to live as independently as possible. This is a full time role based at St Mary's Community Health Campus.
Your new role
The role focuses on promoting independence through rehabilitation, supporting service users from hospital discharge to ongoing care. It involves assessing progress, updating care plans with the MDT, and coordinating further support via Adult Social Care. Daily tasks include assisting with mobility, personal care, and community access, while ensuring safe practices and accurate documentation. The position also includes supervising Rehabilitation Assistants, providing senior cover when needed, and working both independently and collaboratively to maintain high standards of care.
Who is the person summary:
You must have experience of working with adults in a rehabilitation setting, as well as providing personal care. You should be able to demonstrate a clear understanding of the impact that disability can have on individuals' independence. You must be flexible and be able to maintain confidentiality, as well as excellent communication skills. The ideal candidate will be an excellent team player and have strong collaboration skills to help enhance our service.

Salary range
