Job Description
Job Title: Health & Care Improvement Officer
Salary: £45,624 - £52,038 per annum
Grade: F
Contract: Fixed Term Contract (2 years)
Full-Time: 37 Hours per Week
The role is responsible for the development and implementation of the Adult Social Care transformation programme - as part of Better Care Together Thurrock and the Integrated Care Strategy.
The role sits within Adult Social Care - but working in partnership with Housing and local NHS and community partners.

Experience and skills include:
- A good understanding of health and social care.
- A good understanding of communities and voluntary sector agencies.
- Experience of delivering change initiatives.
- The ability of working in partnership with others.
- A Standard Disclosure and Barring Service (DBS) check is required.
