We are looking for a motivated and enthusiastic Health & Wellbeing Coach to join our Health & Wellbeing Team that will be focussed primarily on Healthy Weight interventions. These are evidence-based, and designed to support individuals through behaviour change, nutrition and physical activity improvements. This role includes supporting people living with long-term health conditions to safely make lifestyle changes, improve their wellbeing, and achieve positive outcomes, whilst helping people to develop the knowledge and confidence to manage their health independently.

You will:

  • Provide health coaching support in group settings and/or on a one-to-one basis.
  • Promote healthy weight, physical activity, mental wellbeing, and resilience through positive behaviour change.
  • Support the Health & Wellbeing Team in raising the profile and awareness of the range of interventions available through our service across Blackburn with Darwen.
  • Keep up to date with interventions and services outside the team to enable signposting and referrals.
  • Manage a caseload, monitor progress, and accurately record outcomes and service data.
  • Work with residents from a wide range of backgrounds to help reduce health inequalities through accessible, community-based healthy weight programmes.

We are looking for someone who:

  • Is passionate about working in health & wellbeing.
  • Has practical experience of working with people (either 121 or in groups) in community settings around health & wellbeing.
  • Is qualified to Level 2 (or equivalent) relating to health and/or nutrition.
  • Is reliable, approachable, flexible and have excellent communication skills. The ability to speak community languages is an advantage.
  • Has the ability to travel to venues across the borough to fulfil service delivery, including transportation of equipment.
  • Can use computers to maintain accurate records and enter people’s personal and health data on our web-based client management systems.
  • Enjoys being part of a team; yet possess the personal management skills to work alone.
  • Is highly organised, motivated and resilient.
  • Can be flexible and willing to develop new skills through training opportunities to support the deliver of different aspects of the service, as required, including community health checks and wider health improvement initiatives.
