We’re looking for a Falls Prevention Care Co-ordinator to join our new falls prevention Adult Social Care team in Reading.     About the Role:
    We’re looking for a dedicated and proactive Falls Prevention Care Coordinator to join our Adult Social Care service. In this vital role, you’ll work closely with residents, carers, and a range of health and social care professionals to identify those at risk of falling and coordinate interventions that reduce that risk.

    Falls can have a significant impact on a person’s independence and confidence — and your work will be key to helping individuals stay active, safe, and connected.

    The role involves visiting individuals in their homes, delivering training sessions within the local community, and being willing to undertake falls prevention training as part of ongoing development.

    Your responsibilities will include:

    • Identifying and supporting adults at risk of falls through assessments, reviews, and targeted interventions.
    • Coordinating and monitoring falls prevention plans in partnership with health professionals, occupational therapists, and community organisations.
    • Providing information, advice, and signposting to help residents access relevant support services.
    • Promoting independence, mobility, and wellbeing through early intervention and education.
    • Maintaining accurate and up-to-date case records in line with statutory requirements.

    About You:
    We’re looking for someone with excellent organisational and communication skills, who is confident working both independently and as part of a multidisciplinary team. You’ll have a compassionate approach, strong problem-solving skills, and a genuine commitment to improving quality of life for older adults and those living with long-term conditions.

    You should have:

    • Experience working in a health or social care setting (e.g. as a support worker, care coordinator, or similar role).
    • A good understanding of the needs of older people and the principles of falls prevention.
    • The ability to build trust and rapport with individuals, families, and partner agencies.
    • Good IT skills and the ability to maintain detailed and accurate records.

    You’ll also need to be:

    • A strong communicator with excellent interpersonal skills.
    • Able to work effectively as part of a multidisciplinary team.
    • Confident using IT systems for recording and managing information.
    • Passionate about delivering high-quality, person-centred care.
