Job Introduction

Occupational Therapist

2 x 37 hrs fixed term contract for 12 months

We are seeking two experienced Occupational Therapists to help shape our Making Prevention Real Triage Team in its pilot stages.

The service will operate as part of the front door of Adult Social Care, providing a coordinated, multidisciplinary triage function to support timely, proportionate and strengths-based responses alongside enhancing flow through Family Connect.

Through closer partnership working, co-location arrangements and strengthened referral pathways, the pilot will support earlier intervention, better use of community assets and a more joined-up experience for residents.

Where intervention is required, the multi-disciplinary team will provide short-term, targeted support focused on stabilising or resolving presenting needs. This may include therapy-led input, provision of assistive technology, short term care to maximise independence, joint visits, or connection to Health, community and preventative services.

You will be joining a forward-thinking local authority that was proudly named Council of the Year 2025 and received a Good rating in the 2024 CQC Inspection of Adult Social Care. Our service is recognised for its innovation, including the introduction of Hybrid Practitioner and Family Carer Occupational Therapist roles, as well as the development of the Virtual House initiative.

We are seeking to appoint enthusiastic and experienced Occupational Therapists to deliver a Making Prevention Real pilot service at the front door of Telford and Wrekin Adult Social Care.

Working within a multidisciplinary team and adopting an OT First approach, this is an exciting opportunity to utilise and develop your skills while showcasing the full range of occupational therapy interventions. The role focuses on prevention, early intervention, and enablement, helping to prevent the need for formal care packages, improve outcomes for individuals, and promote independence wherever possible.

As part of this innovative pilot, Occupational Therapists will have the opportunity to shape and influence service delivery, leading occupational therapy practice and championing strengths-based approaches across the team. Successful candidates will be confident decision-makers who can apply professional judgement, identify creative solutions, and work collaboratively with colleagues and partner organisations to achieve positive outcomes.

Applicants should have experience in moving and handling, alongside a good understanding of proportionate care. An important aspect of the role will be assessing and identifying the appropriate level of care and support required when necessary, ensuring interventions are both person-centred and least restrictive.

This role would suit Occupational Therapists who are passionate about prevention and innovation and who are motivated by the opportunity to make a meaningful difference at the earliest stage of an individual's journey with Adult Social Care

You must be a qualified Occupational Therapist, registered with HCPC.
