Early Years SEND Practitioner

We are seeking an individual with the passion, commitment, and drive to make a positive difference to the lives of pre-school children with Special Educational Needs and Disabilities (SEND).
The role involves providing advice, guidance, and support to parents and carers of children aged 0–5 years with SEND across Redcar and Cleveland.
You will facilitate, WellComm and Portage assessments to identify targeted next steps to support children’s progress and development.
Working collaboratively with parents and carers, early years settings, schools, and other professionals, to plan and coordinate each child's educational pathway.
The role also includes leading and supporting successful transitions into nursery and school settings.
An understanding of SEND, inclusive practice, early identification, the graduated approach, and effective multi-agency working is essential. You will be committed to promoting inclusion and ensuring that children receive the right support at the right time.
An understanding of the experiences and challenges faced by parent carers of children and young people with additional needs is also important, enabling you to provide empathetic, family-centred support.
