Post is temporary covering maternity leave

This is an exciting time to work in Knowsley’s Complex Brokerage Team, that sits within the Whole Life Commissioning team.

An opportunity has arisen for an 18-month fixed term post for an innovative individual who is passionate about ensuring the appropriate care is provided for vulnerable adults in the borough.

As a placement team, our aim is to ensure that high quality services are delivered through effective brokerage outcomes for our residents, facilitating timely discharges from hospital, family homes or placement breakdowns or moving on towards increased independence. You will work with colleagues in Adult Social Care and our providers to find suitable placements to achieve positive outcomes for people.

You must be a good communicator with the ability to work in a busy environment and manage your time appropriately often managing lots of different cases at one time, being an influential part of the team. You may be involved in multi disciplinary meetings to fully understand what placement needs a person has.
