About the role

Applicants must hold a valid Driving Licence and have access to a car.

Please note: This role does not meet the legal requirements for visa sponsorship under the Skilled Worker route and so we cannot accept individuals on full-time sponsorship.

We’re excited to be recruiting to our newly created Access Team – the “front door” into Adult Social Care.

This is a unique opportunity to be part of a dynamic service that provides fast, short-term support to people when they need it most.

What will you be doing?

As a Community Support Worker in the Access Team, you’ll play a vital role in delivering timely, strengths-based interventions to adults with a range of needs, including mental health, physical disabilities, learning difficulties, and complex social situations.

Working in a fast-paced, responsive environment, you will:

  • Provide early intervention and short-term support to prevent escalation of needs
  • Act as a key contact at the “front door” of Adult Social Care, helping people access the right support quickly
  • Promote independence, wellbeing, and community-based solutions
  • Help reduce the risk of hospital admission or long-term care placements
  • Work closely with individuals experiencing challenges such as budgeting issues, hoarding, supporting with debt-management, organising paperwork and maintaining safe living environments
  • Collaborate with partner agencies and community services to build strong support networks

The three conversation model paving a new way of working in adult social care and we want Community Support Workers who are enthusiastic and passionate to work with us on this journey. Post holders may work with any combination, or all, of the following care groups: older people, people with physical, sensory, cognitive impairments and people with mental health needs

About you

To succeed in this role, you will bring:

  • Experience of working in community settings, supporting adults with a range of needs
  • A good understanding of real-life challenges such as budgeting, hoarding, and maintaining independence
  • A proactive, flexible approach and the ability to work effectively in a busy, fast-moving environment
  • Strong communication skills, with the ability to build rapport and work collaboratively with individuals and partner agencies
  • The ability to write clear, concise, and accurate records

You’ll also have:

  • GCSEs (or equivalent)
  • NVQ/QCF Level 2 in Social Care (or equivalent)
  • Proven experience in health, social care, or a related field
