    What is the job role?

    Community Case Worker – Adult Social Care LDPI Team

    An exciting opportunity has arisen to join our dynamic Learning Disabilities and Autism Promoting Independence Team as a Part-Time Community Case Worker.

    The Learning Disabilities and Autism Promoting Independence Team work with people who are new into adult social care. We carry out functional and care act assessments; with the aim to promoting a person’s skills and independence levels.

    This is a varied and rewarding role that offers an excellent opportunity for those looking to gain a wealth of experience in adult social care.

    • We work as part of a multidisciplinary team comprising of Occupational therapists’ community case workers, travel trainers and outreach provider services.
    • We support people to promote their independence through reablement, provision of aids, adaptations, picture communication charts and linking people in with relevant universal and community services.
    • You will develop a strong understanding of the services we provide and make a real difference in the lives of Coventry residents.

    This post is exempted under the Rehabilitation of Offenders Act 1974 and as such appointment to this post will be conditional upon the receipt of a satisfactory response to a check of police records via Disclosure and Barring Service (DBS).

    Who are we looking for?

    We are seeking a motivated and compassionate individual who thrives in a fast-paced environment and is ready to take on a rewarding challenge. As a key member of our Learning Disabilities and Autism Promoting Independence Team, you will need to be self-driven, resilient under pressure, and committed to delivering high-quality support to Coventry residents.

    In this role, you will engage with individuals through face-to-face contact, telephone conversations, and collaborative work with a range of professionals, including; Outreach services, occupational therapists, police, housing services, healthcare practitioners, and voluntary organisations. You will be responsible for assessing risk, developing personalised assessments and support plans, and ensuring the delivery of cost-effective care solutions.

    As a Community Case Worker, you will adopt a strengths-based approach to decision-making, promoting independence and wellbeing through tailored care planning. You will also provide signposting to local services and consider the needs of informal carers. A good understanding of the adult social care system and a proactive, problem-solving mindset will be essential in achieving positive outcomes for those you support.
