Job Introduction

We are delighted to offer threefull time posts for the role of Community Care Officer, (CCO), within our Lincolnshire Conversation Team. The areas we cover are; Lincoln, Gainsborough, Hykeham and surrounding villages.

A Community Care Officer level 2 (CCO) plays a vital role in supporting adults with care and support needs, ensuring they receive the right support to live independantly. This role is central to delivering statutory duties under the Care Act 2014,

About the role

As part of the Lincolnshire Conversation team, you will work with people who require guidance and support. Our goal is to support people to achieve their aspirations, access their communities, and promote independence. Working in a person-centred, strengths-based approach, you will complete Adult Needs Assessments and Reviews in line with the Care Act, focusing on what individuals can do rather than what they cannot do.

You must have access to a vehicle and be able to travel efficiently within reasonable timescales to meet service requirements. This may include responding to unplanned visits at short notice, which could involve travel outside the local area or county. Mileage expenses are reimbursed for travel from the office to appointments.

Our core office hours are 08:45–17:15 (Mon-Thu), 08:45–16:45 (Fri). We work a mixture of home based and office working. Currently we work from offices at a minimum of two days per week, however, there will be a requirement to be in the office when required to meet the needs of the service.

The Lincolnshire Conversation Team West are a creative, innovative, passionate team who want to provide the best service we can to the people in our area. We expect our team members to take great pride in their work when supporting people and providing guidance/support to their families. We work within the ages of 18 to 106! Our offices bases are in Lincoln, Hykeham and Gainsborough.

You will be given a nominated contractual work base and you will be required to commute to this work base when necessary. Salary range

  • £32,597 - £35,412 per year
