Salary: £33,531 - £38,052 per annum

Fixed-Term Contract 12 months

Hours: 37 hours per week on a shift pattern consisting of earlies, lates and day shifts

An opportunity has arisen for Care Coordinator to work in the Caring for Thurrock Community Team. The role is to work alongside team members who are responsible for managing the staff and service users within the service. Caring for Thurrock is the internal care department of Thurrock Council Adult Social Care and is responsible for provision of Care for Thurrock residents. The team is a Multi Disciplinary Team working alongside colleagues within the Health Service in providing long term and reablement care to service users within Thurrock.

We provide care and support for clients in their own homes via single and double handed calls. The service provides care 16 hours a day throughout Thurrock – 07.00-23.00, 7 days per week.

Full Driving Licence and access to your own vehicle required for working in the Community – mileage paid at 45p per mile.

This role is mainly responsible for but not restricted to:-

  • Assessing and reviewing service users who have been assessed to receive a care package
  • Managing staff cover
  • Spot checking and supervising staff
  • Supporting with the day to day tasks of the Residential setting

You must be able to work remotely and from sites within Thurrock as necessary to support the service. You will need to be able to collate information within Office 365 systems, spreadsheets etc and attend regular meetings with internal and external customers. Existing experience of using a rostering system is a preference. You will also need to be able to provide hands on care to service users when necessary.

The type of person who will be suitable for this role will need to be organised and committed to meeting the tight deadlines that will be in place.
