An exciting opportunity has arisen within the Adult Social Care and Health Brokerage Service for a Broker specialising in Pathway 3 (P3) hospital discharge referrals from the Royal Derby Hospital. This is a key role in supporting timely, safe discharge for individuals with complex needs who require 24-hour bed-based care on leaving hospital.

Pathway 3 supports people who have experienced a significant life-changing event and who are likely to require long-term residential or nursing care. Working closely with Health and Adult Social Care colleagues, you will help secure appropriate placements in community-based residential and nursing care settings and contribute to Derbyshire’s Discharge to Assess (D2A) pathway.

The role will also involve working together with a team of Brokers with scope for these roles undertaking P3 discharges from the Chesterfield Royal as well as wider Brokering and Duty across the service.

About the Role

You will work as part of an established team of Brokers and will:

  • Source and negotiate short- and long-term residential and nursing home placements for patients identified as requiring Pathway 3 and P2 support.
  • Liaise with care providers, families, hospital teams and social care practitioners to secure suitable, safe and timely discharges.
  • Provide clear and accurate information to partners regarding placement availability, costs, outcomes and service capacity.
  • Maintain detailed knowledge of local care markets, service models, availability and pricing.
  • Undertake some home care sourcing, arranging care packages for people in the community as part of general Brokerage duties.
  • Keep accurate records, prepare brief reports and handle information in accordance with data protection requirements.
  • Use a range of IT systems confidently to track referrals, monitor progress and report outcomes.

Location & Working Arrangements

This post is a fixed, desk‑based role located at the Care Transfer Hub at Clay Cross Community Hospital, working Monday to Friday. There is an expectation of occasional attendance at The QUAD in Chesterfield to support joint working, meetings or service needs.

About You

We are looking for someone who:

  • Has strong negotiation, communication and problem‑solving skills.
  • Can work collaboratively within a busy multi‑agency environment.
  • Maintains excellent working relationships with providers, professionals, and people using services.
  • Is confident using IT systems and able to keep clear, accurate records.
  • Can manage competing priorities and work effectively under pressure.
  • Demonstrates an understanding of care services, personalisation, the Social Model of Disability and the Discharge to Assess pathways.

Experience in social care, commissioning, brokerage, procurement or a similar field is desirable, but not essential. A commitment to excellent service delivery, accuracy and communication is essential.

An NVQ Level 3 in Business Administration or Health & Social Care is desirable, however candidates willing to work toward this qualification will be considered.

Contract Information

This post is temporary until 31 March 2027 due to funding. Extension may be possible depending on future requirements and funding availability.
