
Assistant Social Care Practitioner
We have an exciting opportunity for an Assistant Social Care Practitioner to join our Hospital Social Work Team in Barnsley. We are looking to appoint a highly motivated, enthusiastic, and committed individual to join our established team of Social Workers and Assistant Social Care Practitioners.
The role will entail working with individuals, families, carers, and communities to enable them to make informed decisions, to promote independence, whilst working in a strengths-based approach. Undertaking collaborative working with Health colleagues through the different Discharge To Assess Pathways is also a key part of the role. You will be responsible for completion of Care Act Assessments and construct value for money care packages, and commission and arrange appropriate services. Knowledge of the Discharge to Assess Process is advisable.

What you’ll need
To join us, you’ll need a relevant Level 3 Qualification in Health and Social Care. You’ll bring some relevant experience of working with adults, have knowledge and experience in producing quality assessments and experience of collaborating with various agencies. We are looking for someone with further knowledge and experience on the Discharge to Assess Pathways in Barnsley. We’ll expect you to be a good communicator with a flexible, creative approach; determined to make the most of this unique opportunity.
Salary range
- £31,537 - £34,434 per year
