Our Mental Health Adult Social Care Service is looking for a passionate and dedicated Assistant Practitioner to join our amazing team and help make a difference today.

About the Role:

As an Assistant Practitioner within Southwark’s Mental Health Reablement Team, you will play a key role in delivering short-term, targeted interventions that prevent and reduce long-term social care needs while promoting independence and recovery.

You will:

  • Support Social Workers and Occupational Therapists to implement tailored support plans
  • Work directly with service users to help them achieve their personal goals
  • Deliver interventions in the community, at our team base, and in service users’ homes where appropriate
  • Promote wellbeing, independence, and recovery through strengths-based practice
  • Have opportunities to develop your skills, confidence, and potentially manage your own caseload

About You:

We are looking for someone enthusiastic, creative, and committed to person-centred practice. To be successful, you will have:

  • A good understanding of reablement principles
  • Knowledge of the impact of mental health issues on individuals and their daily lives
  • Experience supporting service users in a person-centred way
  • Experience implementing or contributing to support plans
  • Strong communication and interpersonal skills
  • The ability to work flexibly across community settings

About the Team:

The Mental Health Reablement Team is a short-term service dedicated to promoting recovery and preventing the need for longer-term social care support. We are a collaborative and supportive multidisciplinary team focused on achieving positive outcomes for residents.
