
Assessment & Enablement Worker
About the role
We are looking for an enthusiastic and dynamic person to join The Care Home Review Team in North Northants Council (NNC).
You will be responsible for managing a caseload of individuals; assessing social care needs; developing, co-ordinating and reviewing care support, as well as ensuring that we meet our duty to safeguard vulnerable individuals for people who need 24hr care and support in either a Residential or Nursing Home.
We are looking for people who believe it is important to ensure people have a choice and control over their own support needs. We want people who are committed, like us, to focus on the person, their strengths and networks rather than routinely assessing people for services and eligibility.

What will you be doing?
- Supporting people who live in care homes who are funded by, or are eligible for funding for their care from North Northamptonshire Council. It is essential that you are able to drive for this role.
- Completing Care Act Assessments and Reviewing placements in 24hr Care in line with the Care Act 2014.
- Writing Care and Support Plans.
- Completing Continuing Health Care Checklists and supporting our NHS colleagues with Continuing Health Care Assessments
- Maintaining case records, ensuring information is accurate and concise.
About you
You will have experience of working in Adult Social Care and will hold a minimum qualification of an NVQ / QCF Level 3. You will need an understanding of the needs of vulnerable people and a working knowledge of the Care Act 2014.

Salary range
- £34,434 - £36,363 per year
