So whether it's in person, on the phone or even in the chat group there is always someone on hand to help. </span></p><p><span>You will be working across the Acute ward, A&E, Community Hospitals, and assessing people in their own homes; providing advice and guidance to those people who access or need the service. As well as assessing you will be completing plans, implementing and monitoring care packages, although there is a focus on short term support you will also be carrying out reviews for long term support. Assisting the Head of Legal and Democratic Services, you will provide an efficient and economic legal service to the council in all matters pertaining to property law and practice.
