The team has a blend of community based Occupational Therapists and Community Care Officer Trusted Assessors who complete specialist housing needs assessments to identify major adaptation solutions to enable a person to have better access to the facilities within their homes. We explore personal strengths, their family and friends’ support networks, and using expert knowledge we connect people with things that will help individuals and families to get on with their lives, achieving prevention, reducing or delaying the need for statutory provision of care services.</p><p>Housing Solutions is a county wide service and whilst we patch work within geographical areas, we may at times ask the team for flexibility to cover a wider area depending on the needs of the service.</p><h4>About the role</h4><p>Community Care Officer Trusted assessors will<strong>:<br><br></strong></p><ul>\n<li>Conduct holistic, person-centred assessments during home visits.</li>\n<li>Recommend both minor and major housing adaptations.</li>\n<li>Possess a thorough understanding of The Care Act and Housing Legislation.</li>\n<li>Connect individuals with local community resources.</li>\n<li>Operate in a preventative and enabling manner, fostering independence within homes and communities.</li>\n<li>Build strong relationships with district and borough council adaptations teams.</li>\n<li>Working on your own initiative as well as working as part of a team</li>\n<li>As part of your probation period, you will complete a level 4 accreditation of major home adaptations</li>\n</ul><h4>About you</h4><p><strong>Essential:</strong></p><ul>\n<li>Full driving licence</li>\n<li>Understanding of The Care Act and Housing Legislation</li>\n<li>Experience working in a social care setting</li>\n</ul><p><strong>Desirable:</strong></p><ul>\n<li>Previous experience minor or major home adaptations</li>\n<li>Completed functional assessment course of equipment provision</li>\n</ul><p><em>We welcome applications from candidates who currently reside in the UK with established proof of right to work documentation.
