We want you to work in a dynamic creative service, delivering support to the citizens of Hertfordshire.</p><p>0-25 Together Service is a specialist service for disabled children, young people and their families. Our focus is to ensure we support people to make positive decisions in their lives, enabling their independence so that they can take part in their local community.</p><h4>About the role</h4><p>Joining us as a Preparing for Adulthood Practitioner, you’ll be working with young people and their families and carers from the age of 14 to 25. No day is the same. You will manage a caseload and work with a range of professionals involved in a young person’s life.  You will be expected to conduct assessments of their needs in line with relevant legislation and review packages of care, work collaboratively with other professionals include, school, colleges, health professionals and SEND. Therefore, you’ll need to be able to work under pressure and be extremely well organised, taking a pro-active approach to your workload.</p><h4>About you</h4><p>No previous experience is necessary, but you will need knowledge of social care, good communication skills and have a driving licence and access to a vehicle for work purposes.
