Opportunity for you:
Barnsley Adult Social Care has an exciting opportunity for the position of advanced practitioner.
We are looking for someone to work alongside the team manager in supporting the locality team. The team works with people over the age of 18 in the community. The majority of the work in the team is to complete Care Act Assessments incorporating the Mental Capacity Act, DOLS and court work, along with supporting with Continuing Health Care Assessments. We are a busy team and work collaboratively with our health partners in the community as well as the third sector and housing.
What you’ll need:
If you are great at -
- Supporting a team
- Making complex decisions
- Carrying out reflective supervision
- Working alongside other practitioners and other team managers
- Change management
- Working at an advanced social work level or in an advanced practitioner role.
- Making sure key decisions are made promptly
- Leadership skills and in-depth knowledge of working with adults and adult social care
- Managing attendance, recruitment/coaching.
You will also need knowledge of:
- The legal frameworks that guide and influence the work of Adult Social Care.
- The Discharge to Assess procedures,
- The Care Act 2014
- The Mental Capacity Act 2005
- The DoLS/LPS.
As part of this role, you will need to apply the legislation ensuring accurate advice is given.
A multi-disciplinary approach is a key part of the role, including considering integrated care systems. You will need knowledge of all the work being done across the service, including with the reablement, mental health, the locality teams, alongside the hospital and the D2a process.
