This will include working as part of a dedicated and experienced team to deliver the Active Medway programmes to people who are living with a long-term health condition as well as promoting the benefits of physical activity to community partners and educating various groups from the general public to health professionals about the importance of being physically active for good health.  </p><p>The delivery of this work will vary between face to face and virtual. It will include the delivery of group exercise classes, one to one assessment, physical activity projects, group education session and presentations within community settings and local activity facilities.</p><p>Duties of the role will include supporting the delivery of key community physical activity projects which encourage residents to lead more active and healthy lives.</p><p>The post holder, as a part of the Active Referral Service, will need to have a wide knowledge of physical and mental health conditions and how to advise appropriate activity, safely and effectively, on an individual basis and in a group. </p><p>The post holder will maintain regular contact with service users to establish trust and rapport, and conduct basic assessments, along with agreeing goals to ensure the best outcome for them and to help maintain lifestyle changes.</p><p>To develop an understanding of medical conditions which will require a high level of awareness and sensitivity when making lifestyle recommendations to individuals. Acknowledging peoples' barriers and helping to effectively influence their behaviour change. </p><p>The postholder will develop health promotion material to promote the benefits of physical activity and dangers of inactivity, to empower individuals and communities to have more control over their decisions that influence their health and wellbeing. </p><p>This will involve supporting the work of the Physical Activity Alliance, the Supporting Healthy Weight Network and Whole Systems Obesity by encouraging networking and engagement of individuals and organisations in Medway who have an important job of supporting and empowering the public to make healthy lifestyle choices.</p><p>The post holder will keep up to date with the latest evidence-based guidance on Physical Activity and Exercise from key government documents and key nationally recognised organisations.
