Our First Contact Team is the front door for information, advice, and support to our community, we work in conjunction with the Community Health and Social Care teams and consist of district nurses, social workers, rehabilitation therapists, occupational therapists, mental health workers, learning disability workers. We work in partnership with other health and social care teams such as specialist nursing or children services. This integrated way of working allows us to deliver high-quality person-centred services that meet the needs of the residents. Our services are innovative and rewarding for both clients and the workers
We are looking to recruit an enthusiastic and highly experienced individual to step into this key role, who is:

- Committed to ensuring continued support of adults with care & support needs including their carers
