We are looking for an experienced Care Manager to work with us in our Adult Social Care teams who are leading the way in health and social care integration. This is a fixed term contract until 31 March 2026.
About you…
- Educated to A Level/NVQ3 standard or equivalent minimum.
- Previous experience in an Adult services setting
- Understanding and implementation of Care Act, Mental Capacity Act and other relevant legislation
- Able to complete social care assessments
- Experience of working as part of a multi- disciplinary team
- Manage a caseload and prioritising needs, risks and appropriate signposting
About the role…
As the Care Manager, Contact and Response Team, you will be responsible for the following:
- Facilitate efficient assessment of Rutland Adult residents
- Ensure safe, timely and effective crisis intervention and planned work through the completion and coordination of assessments
- Completion of Needs assessment, Mental Capacity and relevant health and social care referrals.
- Commission services and support in response to assessed need
- To promote and contribute to the safety and well-being of all people in the community through the implementation of the Rutland safeguarding adults’ practices and procedures and multi-agency risk management.
- Maintain accurate and timely assessment and case records. Monitor and review support plans to ensure ongoing effectiveness
- To work in an integrated way with partners including health and colleagues across LLR and locally
In addition, we can offer manageable caseloads, with reflective peer practice, CPD sessions, career development and regular 1 to 1 supervision.
The successful applicant for this post will be subject to an enhanced check by the Disclosure and Barring Service (DBS). The costs for the DBS are paid by us.
The Care Manager Interviews will take place on the 27th June 2025. Please note this could be subject to change and we will give the appropriate notice.
