‘Staying Close’ is being promoted across a wide range of Local Authorities to support care experienced young people who are moving into more independent living. The practice model sitting behind Staying Close is based on positive relationships and access to accommodation and the right support at the right time. In Wirral the support is enhanced by specific mental health access for young people, alongside the ability to sustain relationships with professionals for the long term, particularly for the most vulnerable young people without pre-existing family support.

The Staying Close team is funded for 2 years and is aimed at identifying young people who can most benefit from targeted mental health support, enhanced and intensive ongoing support, as well as a keen focus on achieving the right home for the young person.

We are looking for three Personal Adviser Staying Close : You will be working closely with young people and ensuring relationships are built and nurtured, and engaging in specific work around mental health and emotional well-being to support young people in collaboration with commissioned mental health services.
