Can you work collaboratively and effectively with individuals, building on good partnership working with health colleagues?
In this varied, challenging and rewarding role, you will work with both Health & Social Care colleagues to provide an excellent Social Work service. In West Sussex we have a real focus on strengths- based approaches and community led support. Both you and the team will be working hard to support people to regain their skills and independence following their hospital admission wherever possible.
The successful candidate will have experience of completing Mental Capacity Assessments and Best Interest Decisions under the Mental Capacity Act.
