Team Lead - Carer Health & Wellbeing Project

We are currently seeking a Health & Wellbeing Project – Team Lead to join our team in Medway. The requirements of the role will include working across all GP surgeries within the 8 Primary Care Networks (PCNs) in Medway. You will be leading a small team and be responsible in implementing the delivery of the project in identifying Carers and offer Health Checks withing the GP surgeries by accessing the EMISS database.
To be successful you will have experience of working with adults either in a 1-1 or group setting, a working knowledge of health and social care particularly in relation to adults and have excellent communication and interpersonal skills. A high standard of written skills is needed, in order to help the Carer identify their support needs and build resilience.
The location for the position is flexible but candidates must be willing to work out of one of Carers First’s main offices.
