Our Health, Leisure and Tourism Services Team at Blaby District Council is committed to providing a first-class service to our residents by delivering programmes and interventions that aim to improve their health and wellbeing. The team is now also responsible for delivering physical activity and health improvement services on behalf of Oadby & Wigston Borough Council.
Under this new partnership, we are looking to recruit an Active Blaby Referral Co-ordinator who will be responsible for co-ordinating and facilitating our Active Blaby Physical Activity Pathway, receiving referrals and signposts for residents from health professionals and accessing a suitable activity in the community.
As our Active Blaby Referral Co-ordinator, you will be actively working with residents to support them, develop an active lifestyle as they come through our Active Blaby Physical Activity Pathway. You will use your motivational communication skills to help residents develop plans for themselves to be more active and maintain a caseload to keep in contact to support achievement and measure outcomes. You will also be integral to the continued roll out of our innovative and unique Active Blaby programme which is a web-based system that identifies the most effective behaviour change techniques, determined using the COM-B model, for residents accessing our services by answering a short behavioural questionnaire. This is the central tool to the Physical Activity Pathway.
This role will involve you liaising with and presenting to current and potential referring organisations as well as delivery partners to build capacity and opportunities, maintaining thorough and accurate records of participant progress and evaluation data, developing promotional resources and creating innovative reports for a variety of stakeholders.
This role will require an enhanced Disclosure and Barring Service check (known as a DBS check). All offers of employment will be made conditional upon receipt of the satisfactory DBS check.
