Senior Rapid Response Officer
The Rapid Response service are looking for a senior officer (Senior care worker) to work in a multi-disciplinary team of professionals working in delivering a 24hr response service to avoid hospital admission and to facilitate hospital discharges , you would be managing a team of assistant rapid Response staff this would include rota management and co-ordinating the assessment and triage process managing MDT’s meetings. The service is open Monday to Sunday 8am till 10pm 365 days including Bank Holidays.

Experience in care sector and a Level 2 in Health and Social Care desirable.
