Job Title: Social Worker/Occupational Therapist– Early Intervention Practitioners
Qualifications: Occupational Therapist degree or Social Work degree 
Location: Adult Care Services & CLCH sites in Hemel Hempstead 
Starting Salaries depending on grade:

Level 10 £33,820 with the opportunity to progress to max £37,261 (Social Workers/Occupational Therapists)

Level 11 £38,296 with the opportunity to progress to max £42,503 (Social Workers/Occupational Therapists)

Hours:  08:00 - 18:30 working a maximum of 4 days a week 

1 weekend duty in 4 with 2 days off in the week when working a weekend.  

Contract: Fixed term contract or secondment for one year with the possibility of extending or securing a permanent contract in the wider Adult Social Care department.

New Early Intervention Vehicle Service Hertfordshire Adult Social Care Services are looking for a dynamic, forward thinking Occupational Therapists or Social Workers to work in the newly established Emergency Intervention Vehicle Service supporting Older People in West Hertfordshire.  

This new and exciting initiative is well suited to assertive professionals, looking for a unique challenge, with sound crisis assessment skills and a passion for working with clients to optimise their wellbeing in their own home.  

You will support people living in West Hertfordshire in vehicles provided by Central London Community Healthcare NHS Trust working alongside a Paramedic or Nurse to provide holistic assessments in responding to non-life-threatening priority calls such as falls. You will be expected to take positive risks and work creatively and flexibly to enable older people in to remain in their own home following an acute episode.

You are responsible for managing the immediate concern and considering future preventative interventions, as well as the needs of the carer, before referring for NHS and/or social care support. You will not be expected to case manage cases. You will work closely with the CLCH Rapid Response team and develop strong relationships with Adult Care Services colleagues as well as voluntary services to promote longer term recovery.  Professionals will have immediate access to assistive equipment and Social Workers will receive Functional Assessment training. Each job is managed on an individual basis to ensure the best quality client centred care is provided. 

The team is supported by a band 7 nurse from CLCH and an Adult Social Care deputy team manager. In addition to these duties the Level 11 practitioner will be required to take responsibility for supervising colleagues and other additional duties supporting the team manager.

You will hold a Social Work or Occupational Therapy professional qualification and a current registration for practice with your professional body. You will have experience in working with the older people and their carers.
