Social Worker
We currently have a number of opportunities in various locations and service areas across the city. The work is varied, and you could be working in Mental Health, Complex Care, Care Homes, Neighbourhoods, Supported Living, Service Development, or Transition.
All vacancies include undertaking strengths-based Care Act assessments, person centred support planning, reviews, multi-agency working, Court of Protection applications, and Tribunal Reports, this list is not exhaustive. You will receive regular supervision to support you in your role, and development opportunities so you can progress in your career.
We require the successful applicants:
- Dip SW, BA or MA Social Work or equivalent / or due to receive qualification within next 5 months following application
- Social Work England registered or due to receive registration within the next 5 months following application
- Applied knowledge of Community Care Legislation and subsequent guidance, including Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983
- Experience of completing assessments and support plans and generating these through information technology
Responsibilities:
- Responsible for safeguarding, supporting and promoting the well-being of adults with care
- Contribute to the operational delivery of an effective Adult Social Care Services
- Report writing
- Plan and co-design individual support plans, with regular monitoring and reviews of these support plans
- Responsible for social work provision
- Effectively manage your own complex and varied caseload whilst managing the reputation of the Department and Council and complying with national and service procedures and guidance
- Prepare and participate in Performance and Development Reviews
