Starting salary depending on grade:
Level 10 Social Worker - £33,820 with the opportunity to progress to a maximum of £37,261 per annum
Level 11 Senior Social Worker - £38,296 with the opportunity to progress to a maximum of £42,503 per annum
Plus relocation package, £1,000 welcome payment* terms apply & lease car* terms apply
Hours: Full time and Part time positions available

This is an exciting opportunity to join the Post Hospital Review Team which supports an integrated approach to managing the care of people who require support on the Discharge to Assess pathway to enable those to regain their independence and achieve their goals and outcomes.
The Post hospital review team initiated 5 years ago to enhance the wellbeing of people entering their social care journey on their discharge from hospital to encourage and promote strength-based assessments to support people to regain their independence,
You will be expected to work one weekend every 8 weeks on a rota with days off and after weekend working. This can be discussed further during interview. You will also be offered a comprehensive induction programme with opportunities to shadow services within the integrated discharge service
You will be required to hold a full British driving license and have access to transport.
Experience and qualification:
Level 10 Social Worker - will be able to demonstrate capability of working autonomously and evidence of building working relationships with external partners including health and non-statutory services.
To demonstrate good communication and time management skills achieving the outcomes of people that you work with.
Level 11 Senior Social Worker - will be able to demonstrate capability of working autonomously and evidence of building working relationships with external partners

There will be an expectation of a supervisor role.
You will be able to evidence and arrange team activities to meet (daily/weekly/monthly) caseloads performance targets and schedules. As part of this role, you will be Cascading information on organisations agenda to enhance the team’s knowledge and understanding of best practices to help teams understand and support and deliver changes and improvement.
You will support the management and control of budgets to deliver service priorities and to ensure appropriate resources are available.
You will be professionally qualified and/or relevant degree (or equivalent), plus substantial experience OR substantial vocational experience
Up to date and authoritative knowledge of the work practices, systems, processes and procedures relevant to the role, and able to apply this
Detailed knowledge in area of social care, health, and wellbeing specialism including an understanding of broader issues and practices in the sector.
You will work as part of a busy, diverse social care team alongside Occupational Therapists, Community Care Officers, health colleagues and care providers. The team supports people who have recently been discharged from hospital with social care and support needs and the role will also involve visiting people in care homes and their own homes and other locations to undertake assessments.
You will use your professional knowledge and skills to complete innovative social care assessments and create care plans with individuals, carers, and their personal support networks. Therefore, an ability to develop close working relationships with care providers, and other community-based health and social care professionals to ensure people are receiving the right service at the right time is essential.
You will need to be Social Work England registered with a Social Work Degree. It is crucial that you have knowledge and experience of relevant legislation such as the Care Act 2014, Mental Capacity Act 2005, and other social care legislation as you will be managing complex cases.
An understanding of person-centred care and outcome-based practice is important as this is a key principle within Hertfordshire’s Connected Lives Practice to ensure people are supported to maintain and develop their independence.

To improve outcomes for service users, the Integrated Discharge Service supports a 7-day service. You will be asked to part of the service-wide rota to provide cover on consecutive Saturdays and Sundays once every eight weeks and will receive days off during the weeks before and after. There will be opportunities for you to discuss this at interview if your application is successful.
