The Community Resource Team (CRT) is a joint service provided by the Aneurin Bevan Health Board and Blaenau Gwent Social Services. The CRT provides help to adults, (over the age of 18) living in Blaenau Gwent who need support to stay independent, within their own homes.
The CRT offers an improved service that ensures individuals receive the right intervention, in the right place, at the right time, from the right professional. It simplifies the process by coordinating both health and social care needs.
The main purpose of the role is to work with individuals and their families to maximise their well-being and independence, informed by best practice, guidance and eligibility, as required by the Social Services and Well-being (Wales) Act 2014. You will be integral in providing and supporting the integrated operational delivery of all functions of the integrated Community Resource Team within Blaenau Gwent.
Main Duties include: -
- Undertaking social work assessments
- Working with individuals and their families to maximise their well-being and independence
- Provide care planning and care management to service user and their families
Qualifications & Skills: -
- Registered with Social Care Wales
- Diploma/Degree/Masters in Social Work
- Experience of multi-disciplinary working
- Knowledge of current legislation/guidance & regulations in relation to Adult Services
Access to a vehicle for work purposes is a requirement.
