About the Role

To meet the objectives of the Council and its partners a two-year Health Integration Development Officer post has been created to support the development of projects and services, as a system wide approach to health. You will contribute across a range of established functions of the Council including Community Services, Home Improvement Agency, Housing & Homelessness, whole systems approach to obesity, as well as on new projects, service developments and initiatives identified by partners.  It will be your task to forge links with operational teams within all areas of health and care, bringing together stakeholders to contribute to developments and projects, including working closely with partners such as Frimley Park Hospital, Adult Social Care, the GP Federation and community providers to improve care pathways and health outcomes within the Borough.

About You

You’ll need to demonstrate a strong understanding of the role that a local authority has in health and care, having previously worked extensively in either a health, social care, council or other wellbeing environment. The role will require outstanding organisational and project management skills plus a proven ability to deliver successful outcomes through partnership working.
