Social Worker
The Brokerage and Professional Support Team provide services to vulnerable adults who have a learning disability, older people and a physical disability. The team meet the long-term eligible care and support needs of both customers and carers, to enable them to live a safe, fulfilling and independent life. It is a busy yet supportive team, which consists of Social Workers, Occupational Therapists, Sensory Needs Services, Social Care Practitioners, Technology Enabled Care Team, Reviews and Transitions.
We are currently looking for a qualified Social Worker, to join our team. The post holder will work within an established multi professional statutory team and will undertake re-assessments for people in the community with long term eligible care and support needs in line with the Care Act (2014) framework. They will have a high level of knowledge and experience of working with all relevant legal frameworks, such as Safeguarding and the Mental Capacity Act.
The role involves working with adults with a broad range of needs including people with learning disabilities, physical disabilities, older people and people with dementia. It also involves completing Carers Assessments and providing information, advice and support to Carers. We are looking for a motivated, caring and creative applicant to look at ways to enhance outcomes for customers through the strengths-based approach.
The post holder will be a qualified Social Worker and registered with Social Work England. They will have the ability to work in a busy, fast paced environment and to be able to plan and adapt to change is also essential. You will need to be flexible and be able to prioritise and manage your time effectively. We are a supportive and established service, offering a robust induction programme, regular supervision, and good opportunities for CPD training and development, such as BIA training.
