Hours: 37 hours per week / 22 hours per week for Potters Bar Community Hospital
Contract: Permanent

Older People Service - Stevenage

  • Community Review Team - 12 month fixed term / secondment 

The Community Review Team work closely with the Locality teams and Post Hospital Review Teams to identify individuals who would benefit from their intervention to regain independence.  The team work closely with Enablement Occupational Therapists to provide a holistic assessment. 

Hospital East - Harlow 

  • Princess Alexandra Hospital Team

You will work as part of a busy social care team at Princess Alexandra Hospital, completing social care assessments to ensure that Hertfordshire residents are discharged in a safe and timely manner. You will develop close working relationships with the hospital’s Discharge Team and care providers to ensure effective care planning. You will adhere to Hertfordshire’s Connected Lives Practice Principles by ensuring that people are supported to maintain their independence and receive person-centred care and support.

Adult Disability Service - Hemel Hempstead

  • Review team, Watford & Three Rivers & St Albans 

The Adult Disability Service provide support to adults with a learning or physical disability. The team comprises of Team Managers, Social Workers, Occupational Therapists, Community Learning Disability Nurses and Community Care Officers. We are looking for someone who can deliver first class support to vulnerable adults with a physical and or learning disability. You will be Self Motivated, Flexible and resilient in providing excellent team support whilst operating within departmental Policies and Procedures.

Responsibilities for both teams

  • Provides professional responses in situations where people need support, handling complexity and ambiguity dependent on experience. Using analytical skills to inform assessment, decision making and interventions.
  • Completes support planning with service users to ensure that outcomes identified at assessment are met through the provision of support, equipment, adaptations and enabling programmes
  • Actively engages with, assists and provides advice to carers, conducting carer’s assessments, to enable them to sustain their caring role.

Older People Team - Hemel Hempstead

  • Watford team 

This is an exciting opportunity to join a high performing and supportive team working to support citizens over 65 years who live in a vibrant, multi-cultural town. We are committed to working in collaboration with our key partner agencies (health, social care and mental health services). The aim of this role to promote independence by assessing older people to identify their desired outcomes to maximise independent living whenever possible.

You will undertake assessments of need to determine eligibility and implement care and support plans with clear outcomes to achieve or maintain well-being. You will keep the service user at the centre of all activity and address assessed needs within the legislative framework. You will need to be able to work autonomously within agreed timeframes and manage frequently changing situations. You will work as part of an integrated team, attend multi-disciplinary meetings and need to have excellent communication skills.

Hospital – Potters Bar Community Hospital (22 hours) 

  • Herts Valleys Integrated Discharge Service, Community Hospital Team

Working as part of a multidisciplinary team, to identify individuals who may need care and support from Hertfordshire County Council. You will complete Adult Social Care assessments in partnership with individuals and their carers, and arrange care, to enable safe and sustainable discharges from the Central London Community Hospital, Potters Bar. As well as ensuring timely access to assessments and care, you will provide quality advice and signposting and adopt a solution focus approach as part of a multidisciplinary team ensuring that discharges are facilitated in a timely way. 

This role is an integral part of the service. We need someone who can adapt, be effective and efficient in their role, enabling them to help improve service delivery, and advocate for the citizens of Hertfordshire.
