The team works hard in a busy environment to support timely discharge with minimal days lost.<br><br>Our team follows the mobile worker model, sharing desk time between home and our office base at the Town Hall, Weston-super-Mare. Most assessment work is carried out in the community in care homes or in people's own homes.
