We deliver person-centred adaptations that meet long-term needs and ensure the best outcomes for our clients. As a team, we support each other by working collaboratively to provide a holistic, timely, and cost-effective service that makes a real difference in people’s lives.
<h3>Key responsibilities…</h3>

<li>Carry out person-centred assessments and work with service users and colleagues to create practical home adaptation plans that promote safety, independence and enable individuals to live safely in their own homes.</li>
<li>Contribute to capacity building across teams by sharing knowledge of major adaptations, legislation, eligibility criteria, and associated processes, while actively participating in multidisciplinary meetings and case discussions where housing needs and adaptations are considered as part of wider care and support planning.</li>
<li>Maintain accurate, up-to-date electronic records, ensuring that assessments, decisions and outcomes are clearly documented and write clear, evidence-based reports and professional recommendations to support DFG applications and specifications for adaptations.</li>
<li>Make timely referrals and signpost individuals to relevant services, community support or other specialists, when needs fall outside the DFG scope or where additional support is beneficial.</li>
<li>Actively engage in supervision, performance discussions, and reflective practice while operating within professional competence, seeking support when needed, and committing to ongoing training and development.</li>
<li>To ensure all practice complies with the policies, procedures and standards of the council, as well as the Health and Care Professions Council (HCPC) Code of Conduct and Standards of Proficiency.</li>

Team Objectives: The Learning Disability and Neurodiversity (LDN) Team works closely with other teams in Adult Services who are involved with the delivery of Adult Social Care. We work closely within individuals from transitions to adulthood and older age. We promote a strength base ethos to other the best outcomes for the individuals that we work with. Our aim is to be a high performing, efficient, multidisciplinary team with a clear Team purpose and values that supports staff and values the skills and diversity within and around the team. You will hold a varied caseload and contribute to the team performance and positive outcomes for our residents. This post offers opportunities to improve your skills and experience in Adult Social Care with high quality, client-led, interventions to service users in their own homes, in a friendly and supportive team. This is a full-time post offered as either early/late shift on alternate weeks or compressed over 4 days. 
Early shift is 7.40am to 4.00pm, late shift is 9.55am to 6.15pm with an hour for lunch break.

We have an exciting opportunity available for Instructors to join us as key members of the Dinton Activity Centre team here at Wokingham Borough Council, on a casual basis.
Dinton Activity Centre is a thriving local authority centre based with Dinton Pastures Country Park. The centre handles a footfall of around 20,000 people per year, so the place is very much alive during peak season. We have a fleet of 30 sailing boats and 90 canoes, kayaks and paddle boards. On land we have a low ropes course, bush craft area, a rock climbing and high ropes facility, two orienteering courses and a mobile climbing tower.&nbsp;
Key Responsibilities:
As an Instructor, you will:

<li>Support the delivery of activities such as taster sessions, school/youth group events, corporate days, school holiday activities and BC/RYA courses</li>
<li>Assist with boat hire, offsite events, centre maintenance, and general administration / reception cover duties where necessary</li>
