We have an exciting opportunity for a Senior Social Worker to work in a dynamic forward thinking team within Wokingham. The team works with people who have a learning disability, physical disability, brain injury, mental health issues, older people with frailty and other health conditions. 
The role will focus on working with existing customers who have been identified as having complex care packages. This will involve working creatively to identify whether needs can met in different ways. The aim will be to: reassess need; provide advice and support with signposting; maximising social capital; collaborate with key teams to provide a seamless service for individuals; and identify themes and solutions. 
You will need to apply a strengths based model of care and also need the skills to be assertive, deliver change, and challenge differences in a respectful manner. 
 
You will have the ability to work with professional confidence, autonomy and take appropriate decisions in a person centred manner. 
 
As a Senior Social Worker, there will be an expectation that you will undertake Supervision for team members and support the Team Manager in the day to day running of the team. 
 
You will also be instrumental in ensuring that effective systems are in place to ensure that we deliver services that improve outcomes for residents of Wokingham. 
 
In addition to being committed to helping adults to improve the quality of their lives, you will need excellent IT skills, good written and verbal communication skills and a good telephone manner.
