We have an exciting opportunity for you to support in leading the development of our new, forward thinking Transitions Team within Wokingham. We are looking to expand and develop a new service, with the aim of providing a more planned, effective and responsive service to children with eligible needs, aged between 14-25, and their carers. We would like you to be part of developing this change. 
 
We would like a Senior Social Worker to work alongside the Team Manager to develop the Transition Team through a change process and establish, maintain and lead local joint working relationships with Education, Health, Social Care and Commissioning. 
 
Your role will involve working alongside children’s and adult’s services to identify future needs for young people between 14-25 years. You will be qualified and professionally registered, with proven experience of effectively supervising and managing others. You will have the ability to work with professional confidence, autonomy and take appropriate decisions in a person centred manner. You will also be instrumental in ensuring that effective systems are in place to ensure that we deliver services that improve outcomes for young people. We are looking for someone who has a visionary approach and is motivated by the challenge of improving and driving forward the Transition Team for the benefit of those it serves.
