The post holder, alongsidethe Pathways Manager will work around clients with Substance Misuse supportneeds to match hostel client records with Reset and Drug Intervention Program(DIP) records and identify those known/and not known to these services tosupport them to move on from the hostel in a structured and planned way in linewith their recovery plan and the assessed move on housing pathway.
The Pathways Team is in theHOST Team within the Housing Options Service providing homelessness prevention,housing options and bespoke service to those vulnerable and complex need singleclients that are either homeless or threatened with homelessness.
The role of this officer isto assist and guide single clients with support needs to make appropriatechoices regarding their housing pathways, ensuring that they receive, eitherdirectly or through relevant partners, appropriate advice on their housing optionsthat offer alternatives to the making of a homeless application.
This role will involveworking closely with the Resettlement officers in the commissioned supportedaccommodation services to ensure clients gain the necessary pre-tenancy skillsbefore moving on. The post holder will work actively with the Drugs and AlcoholTeam services/Hostel providers to ensure that the clients are referred to andare complying with their treatment plans
The team works closely withProbation, The Prisons, the Royal London Hospital, East London Foundation Trustand other partners taking direct referrals following agreed protocols, workingtowards a prevention agenda.
Where prevention is notpossible a needs assessment is conducted to identify suitable housing pathways.
The job requires that theofficer preferably has an SVQ or HNC in Health & Social Care or equivalentat level 3 or above. A recognised counselling or substance misuse qualificationwith the ability to work well under pressure with others and clients withchallenging behaviors or difficult.
Possessing the ability tocommunicate effectively verbally and in writing to a good standard, the postholder is expected to be able to give advice and information sensitively andpersuasively. An experience of working in a busy customer-oriented environment,delivering excellent customer service is required.
The post holder will need tobe empathetic with an eye for detail with a good knowledge of the service thatsuch clients can access will be helpful. Using a dedicated assessment tool todetermine the most appropriate form of supported accommodation for singleclients, the post-holder will be required to provide detailed Notice letters toclients setting out advice given and the agreed housing pathway wherehomelessness cannot be prevented.
