You will work a flexible shift pattern across 7 days a week, Mon-Fri 8am-8pm and Sat and Sun 9am-5pm including bank holidays, with your future colleagues who are friendly and very supportive.

Are you a qualified, HCPC registered social worker who has successfully completed their Assessed and Supported Year in Employment (ASYE). (This applies only to social workers who qualified on or after 2012)

Our role is to work innovatively to assess to discharge residents of Surrey in a safe and timely manner. Working within a multi-disciplinary team that partners with Surrey Heartlands, ICP & ICS, you will benefit from shared practices and development opportunities. Our social workers are excellent communicators, motivated, supportive, flexible, creative and strong team players.
You will need to be confident in undertaking safeguarding enquiries, carrying out assessments within the hospital and also the community, and working with a diverse group of individuals and partner agencies.. You will be expected to have a sound knowledge and experience of current legislation, including the Care Act, Continuing Health Care, and the Mental Capacity Act and its application to practice in a social care setting.
You will be required to supervise less experienced members of staff within the team and support their development. In return for all of your skills, enthusiasm and commitment we will support you fully in becoming the professional you want to be.
