As a Review Officer within Adult Services, you will be key to providing a quality service to the people of Hackney. We will support you to achieve the very best outcomes for our residents. You will have a sound knowledge of the key principles of the Care Act, the Mental Capacity Act and Safeguarding as well as all other relevant legislation relating to the provision of services for adults with long term conditions.
You will have a commitment to carrying out reviews in many differing geographical areas and settings, from older persons at home with an intensive support plan to working with a young person with a Physical Disability or Learning Disability in supported living, residential or nursing care setting.
We would like our review officers to be professionally curious and always be vigilant and to understand the need for step down and rehabilitation. We are committed to a strengths based approach, which includes quality, asset based reviews.
A commitment to working in an integrated manner with health colleagues is essential, as well as an understanding of the value of working with the wider multidisciplinary team such as the Police, Housing, Education and Children’s services.
We are looking for someone who can make a positive contribution to the work of Adult Services as a whole and we will offer you regular individual and group support and supervision to enable you to make the very best decisions and support plans. You will be an enthusiastic proponent of promoting every individual service users independence wherever possible, with a sound understanding of personalisation including use of Direct Payments.
