Single Point of Access Team (SPA):
The single point of access team is the entry point to services. You will work in an integrated multi-disciplinary team and be responsible for over-seeing the Occupational therapy referrals for both health and social care received by the contact centre, Care Connect. You will be responsible for the supervision/management of Occupational Therapists and Adult Social Care Workers, in addition to contributing to service development within the SPA team and wider OT service.
A significant proportion of referrals are resolved at the point of contact through the provision of advice and guidance, signposting and prescribing equipment or minor adaptations, including the use of assessment clinics. You will undertake a specialist role in the development of early intervention and prevention services at our Equipment and Demonstration centre whilst working to increase our clinic offer at the centre and the use of Equipment and adaptations. When resolution is not possible in SPA we will refer to our integrated teams and non-statutory organisations for their services. You will need to be a good communicator, able to build rapport over the telephone and work proportionately and creatively to meet service user needs in a timely way.
