Northamptonshire Adult Social Services (NASS) is on an ambitious and exciting journey, and has recently undergone a major Transformation Programme. We currently have an Assessment and Enablement Worker role within the Short Term Enablement and Prevention Services (STEPS) Team South. We are looking for (HCPC) registered Social Workers who want to be working in a team whose aim is to prevent, reduce and delay the need for on-going services through optimising the self-caring abilities of our customers and to be promoting independence and community living to help minimise the risk of hospitalisation or admission to residential care. The Short Term Enablement and Prevention Service South Team works with a wide variety of adults whom may have mental health problems, physical and/or learning disabilities. As a Social Worker You will be responsible for managing a caseload of individuals; assessing social care needs; developing, coordinating and reviewing care support, as well as ensuring that we meet our duty to safeguard vulnerable individuals. You will be committed to working with customers in a person centred way to support them to achieve effective outcomes. You may also be asked to undertake social circumstance reports for tribunals and present them and undertake Human Rights Act assessments and liaise with our legal services. You need to have good organisational skills, and will be an effective communicator used to working in conjunction with a range of other organisations, including health and other professional colleagues. You will also have a strong demonstrable commitment to working from a strengths based perspective. We are committed to the development of the Social Care workforce.
