We are recruiting Social Care Officers to work within our Locality teams. You will be required to assess the care and support needs of adults, including those with mental health needs, people with physical disabilities, and their carer’s needs, within the community. We are looking for applicants who have had previous experience of working with these user groups and are passionate about supporting people and working with their strengths.
As part of a team you will ensure client and carers’ wishes are central to the decision making and together develop and implement appropriate support plans which will maximise independence and help them to remain living in the community for as long as possible, or assist them to move seamlessly into more appropriate accommodation.
You will be required to work collaboratively with multi- disciplinary teams in the community including the development and monitoring of reablement programmes.
The successful applicant will ensure vulnerable people are safeguarded and promote safeguarding personal values and ethics through day to day work.
